IncrementalMeshHandle

A mesh on its way to the device, and the iterator that gets it there.

The call that returns done is the one that does the last step's work rather than an empty call after it, so a mesh takes exactly as many calls as it has chunks and a caller counting stops against a frame budget counts the right number.
